How to Apply

1) Determine the scholarships for which you are applying by using the grid found by clicking here.

*** NOTE: It is VERY important to follow ALL instructions in the “How to Apply” column on the grid.

2) Complete the application requirements noted in the far right column of the grid found above. Find all application documents here.

3) Hand-sign the documents. 

4) Mail or hand-deliver all required materials to Unity Foundation of La Porte County no later than 5 pm on Wednesday, April 11, 2012.  Applications may be postmarked no later than April 11, 2012.  The office is open M-F from 9 am to 5 pm for hand-delivery.  Late or incomplete applications will NOT be considered.
